W3 ASSIGNMENT (Follow W2 Assignment papers, above).
Health Care Program Evaluation Draft.
Sexually Transmitted Infections Youth Santa Monica Area Program.
FIVE pages
Resource: There are multiple resources that can be used for crafting the program evaluation plan proposal depending on the type of program. One resource that is of value for understanding how national programs are designed and the evaluation plan model is developed can be found from viewing: Healthypeople.gov: Healthy People 2020 initiatives, goals, and measurements
Write a draft of the first three sections of the program evaluation plan to include the introduction, program background, and literature review.
• Introduction: This is not a summary of the proposal; it is a brief statement of the plan’s purpose. Start with a clear purpose statement. Add a description of each major section, as presented in the completed plan. This section must be no more than one-half page in length. Review Ch. 1 of the Posavac text.
• Program background: Insert updated content from your Week Two assignment with revisions based on facilitator feedback. This section must be no more than one-half page in length. Review Ch. 2 & 3 of the Posavac text.
• Literature review: Describe findings to date from a review and analysis of literature. This section must be one to two pages in length.
Format your draft according to the following:
• Title page, including title of study, author’s name, and institution name
• Table of contents, listing headings that are also found in the narrative, and corresponding page numbers
• Table of exhibits listing type, number, title, and corresponding page numbers
• References(4)minimum
Submit the introduction, program background, and literature review sections to your facilitator and your Learning Team forum for peer review by at least two peers in Week Four. View week four for where you are to post your proposal draft for your assigned team.

Tasks
Review Week Eight Expectations for Total References and Word Count Minimum
Review the Number of Peer Reviewed Journal Articles Required by week eight as part of the total references. Note: not all journals are peer reviewed and governmental or organizational reports or website content does not count as peer reviewed. If you are unsure of journals being peer reviewed pull up information on the publication through their website. Note if using library click peer reviewed. Google scholar lists this off to the right. Remember to format the Journals as per APA 6.0 requirements.
• Personalize the Template you are to use for your approved program. • Build your Table of Contents for the entire plan document sections for week three through week eight for level one and level two headings. • Use the resource to learn how to do this automatically. • Make sure you if necessary you change the color to black font to Times Roman 12 and spacing between headings and content appropriately. • You also need to ensure that APA 6.0 is maintained for the level one two headings in the body of the paper as per the template you downloaded. • I suggest you do a save as of your plan document to the program name and your name after it. That way if you have any issues along the way you can look at the original template. • Review the section instructions for the week three submission. You can go over the word or page count if needed. • Do not go under. • You should update and expand on the program background from the week two feedback and if some of the other sections in that document seem to be background, you can include them, but in a narrative paragraph not a cut and paste of a heading and content, please. • If you look at the entire template you will see some of the content such as goals belongs in later week sections. • I also recommend in later weeks that you add to your literature review when you add citations elsewhere through all weeks where applicable and add level two heading in the literature section as warranted to break out content for your leadership audience to justify and support what you are recommending in one place, • Also try for more than one source cited. Note your literature review should be primarily peer-reviewed sources.
