The Internet is viewed as a revolutionary change due to its equalizing potential. It allows every demographic in every reach of society to witness and communicate with any other. At the same time, it allows even the smallest individuals in the least developed nations to access the secrets and touch the vulnerabilities of the most powerful.
Describe some of the attributes of revolutionary change brought about by the Internet and their impacts on security.
Among the characteristics of change introduced by the Internet is cyberwar. Russia has successfully applied cybewarfare in Estonia, Georgia, and the Ukraine. The anonymity of the Internet emboldens cybercriminals and state actors alike. Apparent attribution is quickly made ethereal through contrived explanations. During the 2007 Estonian incident, computers with Russian Internet service provider (ISP) addresses proved to be the origins of the attacks. However, Russian authorities shrugged off accusations, claiming those computers were merely hijacked victims, no different than the bots involved (Ashmore, 2009).
Similarly, China stands ready to attack and respond to attribution with its three warfares. According to Sun Tzu (as cited in Thomas, 2010), “all war is deception” (p. 108). Through the first warfare, China’s state media places the state’s illicit activities in a blameless and virtuous light. The second tasks state-sponsored legal experts with formulating justifications for the country’s intellectual property thefts. Finally, in the third warfare, China charges psychological warfare experts with the duty of bolstering local morale while dampening that of its enemies. Through these warfares, China seeks to save face while buttressing public and international opinion (Thomas, 2010). In a 20 Ma7, 2014 report, China responded to indictments against five of its Army officers for “illegally accessing protected computers, identity theft, economic espionage and stealing trade secrets”, with a formulated and mechanized retort (as cited in Song, 2014, para. 5). Specifically, China ignored the financial-gain aspect, stating that “the indictments are ‘based on deliberately fabricated facts… [The U.S. has] long been involved in large-scale and organized cyber theft as well as wiretapping and surveillance activities…” (as cited in Song, 2014, para. 6). The advent of the Internet and China’s use of state resources to steal from the West and give those gains to its industries have served to quadruple the country’s gross domestic product over the past two decades alone (Noonan, 2012). U.S. academia’s development of the Internet has revolutionized China’s economy at the expense of U.S. innovators. Before the Internet, nations risked physical placement of spies who had to integrate with an organization and develop means of removing documents from a facility, or employ technical surveillance equipment within and above the earth’s atmosphere. Today, a state-sponsored actor can inflict far greater damage from the security of a computer terminal, far from foreign soil.

Revolutionary change often creates a paradigm shift. Given a new paradigm, it would be beneficial to redraw the cybersecurity landscape, critically assessing how the problem of cybersecurity should be defined.

As such, governments and corporations are investing unprecedented amounts of resources in “cybersecurity.” What is cybersecurity? Generally, cybersecurity is a broad and loosely defined term that encompasses a range of information security technologies and services, including identity and access management, breach incident response and the protection of information technology infrastructure such as networks, routers, email and web servers (Bissell, 2013). However, it is not a simple function or task of providing protection to cyber assets. In today’s technology-dependent world cybersecurity is a strategy aimed to preserve our way of life.

Cyber crime and cyber attacks are not the impetuses that drive the need for cybersecurity. Vitality is what drives cybersecurity. Cybersecurity strategies are designed to ensure survival and the continuation of a purposeful existence. Today, every significant public and private activity – economic, social, governmental, and military – depends on the security of information systems. Yet, those systems remain inherently vulnerable (Brenner, 2010). The modern world has become so dependent on information technology that a catastrophic failure could have devastating consequences. However, even a seemingly harmless event could have a ripple effect across an enterprise. That is why cybersecurity is more than a compliance issue (Bissell, 2013). Cybersecurity must move beyond an auditable, check-the-box routine and become an on-going, proactive approach.

A. What is an asymmetric threat?

An asymmetric threat has a complicated etymology. A conflict or war can simply be asymmetric based on one side having more people or resources than the other. Asymmetric wars can also be a difference in approaches or methods of fighting. A modern post 9/11 definition of asymmetric war is related to the the changing role the state plays. Wars are no longer fought by two states with conventional weapons and clearly defined enemies. Today’s threats come from terrorists, proxies, mercenaries, and other non-state actors. There are asymmetric threats from non-state actors who can do great damage to commerce and infrastructure using computers. The internet allows actors that take refuge within a sovereign nation to act on their own accord against other nations using cyber attacks (Ruzo, n.d.; UMUC, 2014).

B. Describe some dynamic approaches for defending against an asymmetric threat.

To create a dynamic approach, there are lessons that can be learned after the 9/11 attacks. The attacks were an asymmetric attack that exemplifies failures of national security. By understanding what part of the system failed, new strategies can be developed and applied to defending against asymmetric cyber threats. The government was emphasizing classification of information over sharing of knowledge which prevented the pieces of the puzzle from being put together. There should be fact finding procedures that can help understand all aspects of cyber threats. A key takeaway from the 9/11 Commision is “We propose that information be shared horizontally, across new networks that transcend individual agencies” (UMUC, 2014).

Defense in depth using risk-based analysis can also help create a dynamic approach for defending against asymmetric threats. This can be useful because a layer of the defense can be training staff to respond to a threat rather than using a technological control. This may be more effective and adaptable.  It is important to keep many aspects of cybersecurity in mind when creating a defensive strategy including policy, training, and technology. Human factor controls will be able to defend against specific threats at one defense layer while technological may work at others  (UMUC, 2014).

The counties of Mitchell and Yancey have the highest poverty rate comparable with the regional and state-wide rates. More than the 20.3% of the population, it is uninsured (Blue Ridge Regional Hospital, 2013).

Blue Ridge Hospital  mission is “ Identify and respond to the health and wellness needs of the region, partnering with patients, families and friends through a comprehensive approach to healing that ministers to the mind, body and spirit” (Blue Ridge Regional Hospital, 2014).  The primary payer of this organization is Medicare, and it is supporting by hospitals and health departments in the sixteen western counties of North Carolina coordinated by WNC Health Network and the western North Carolina Partnership for Public Health. The WNC healthy impact vision is improving the community health and, its priorities are chronic disease prevention and management and, mental health and substance abuse (WNC, 2013).  Blue Ridge Hospital duties and responsibilities are “serving the people as non-profit health system governed by the community, ensuring sustainability through stewardship of the community assets, provide quality service in a compassionate, cost-effective manner, and promote wellness and health to benefit the community” (Mission Health, 2013). Entities that receiving more than $5 million in Medicaid reimbursements, must have detailed written policies; established about the federal and State False Claims Acts for their employees, contractors, and agents. Laws like The Deficit Reduction Act (DRA), Policy #200.060 and #200.164 (Mission Health System, 2013), regulating the Blue Ridge Hospital.  The Blue Ridge Hospital has provided excellence in service since 1995, recognized as a 5-Star hospital. Little by little, and with the support of the community the hospital currently offer the services of emergency, OB, critical care, rehabilitation, cancer center, orthopedics, pain management, pediatric, family medicine and more; dignifying every day the organization mission.

Part 2

Blue Ridge HealthCare (BRHC) is a private, not-for-profit community healthcare system in Burke County, North Carolina. BRHC includes two hospitals – Grace and Valdese Hospitals. BRHC has a Board of Directors drawn from the local citizen volunteers (Bailey, 2012). The Board consists of 17 members headed by the president and chief executive officer (CEO), who directs and coordinates all the activities in the system. The current CEO is Kathy C. Bailey. Under the president, is a vice president – Larry Hiddale. The board also has a chairperson, co-vice chair, secretary, treasurer, and board members.The ultimate responsibility of the BRHC Board is to direct the healthcare system toward achieving its mission and a state of well-being. It has a legal responsibility to govern BRHC and is accountable for everything the organization does. The Board monitors BRHC’s finances and supervises its management. It puts BRHC’s interests above members’ personal interests, and ensures that the healthcare system complies with the law.

The Board of Directors at BRHC has three primary duties – establish policies, oversee the activities of the healthcare system, and make important and strategic decisions about the system (Arnwine, 2002). The Board makes policies that define the focus and responsibilities of the management, board, and medical staff. It also establishes a clear mission, vision, and purpose for the healthcare system. Its oversight duty entails approving the programs of BRHC, ensuring sound financial management and stability, and establishing standards for organizational performance. The Board also holds BRHC accountable for its actions.  The BRHC Board of Directors has a duty of care, loyalty, and obedience. Care means paying attention and displaying due diligence. Loss of money due to recklessness, indifferences, or failure to seek advice places the Board at a legal risk of being sued. Failure to exercise loyalty, by putting individual interests before those of BRHC, can lead to private inurement. Obedience requires the Board to ensure that BRHC complies with the law and all decisions and actions are consistent with its mission, as well as governing documents such as a charter. Complying with this duty protects the board from lawsuits. Overall, failure of the Board to comply with the three duties, fraud and abuse statutes, laws, and patient privacy regulations exposes the whole healthcare system to civil and criminal monetary penalties (Davidson & Murdock, 2013). Patients can file derivative action lawsuits, and donors (like Blue Ridge HealthCare Foundation) may sue the hospitals for alleged misuse of assets or gifts

Part 1

Pre-service activities create value before the actual delivery of care (Ginter, Duncan, & Swayne, 2013, p. 133). These pre-services activities can include marketing, pricing, branding, and/or
promotion (Ginter, Duncan, & Swayne, 2013, p. 133). Therefore, my
expectations of the pre-service activities would be the ability to acknowledge
that the provider exists. In addition, the ability to see prices for basic
services provided and the ability to easily make an appointment. I would not
want to go through numerous prompts in order, to finally talk to someone about
making an appointment nor would I want to talk to a rude receptionist. Point-of-service is activities that occur when the patient arrives to the organization (Ginter,
Duncan, & Swayne, 2013, p. 135). My expectations of point-of-service would
be a clean environment, up-to-date seating and technology, if applicable, no
long waiting times (no overbooking patients to earn more money in a shift),
courteous and friendly atmosphere. After-service activities consist of value
that is created after the patient’s initial visit to the organization (Ginter,
Duncan, & Swayne, 2013, p. 133). My expectations for after-service would be
the organization handling the paperwork for the insurance company (billing),
referral if need to see a specialist, and assistance if needed follow-up
appointments. I have witnessed some organizations that call a couple days after
service to see how things are going, that is not important to me, it is nice
but I don’t expect it. Overall, I would expect a clean, friendly, efficient,
and quality appointment. My time should be treated as valuable as the
healthcare providers. Some factors that would discourage me from using a
healthcare facility in my community would be the location, cleanliness, staff
members, and reputation. I would want the facility close to my residence unless
necessary travel time is required (specialist etc…) and I would want to be
able to enter/exit the parking lot in safe manners. Most importantly, I would
want the staff to be friendly and the provider must have a reputable image
established. I would not go into a dirty restaurant to eat and therefore, I
would not want to receive care from a facility that was dirty either.

Part 2

As a consumer I expect to be treated well, there needs to be a level of respect,  dignity and above all, a sense of professionalism (be it pre-service or after-service). I would expect the visit to go smoothly and I assume to deal with competent staff members. The environment has to be clean and effective (where the equipment is operational and working as it should). After-service I would expect a nurse or staff member to call or write to see how I am doing. My expectation would be to have all the notes, results and/or exam records entered into my medical record file and if needed forward to the appropriate doctors (primary care physician, specialist or surgeon).  I would like to visit a healthcare facility that is known not only for its name but for its reputation and accountability record.
I would reconsider using a facility that was known to have a bad track record such as medical record errors, bad bedside manners, a high record of infection, or mis -dosage of medication. Unless absolutely necessary I would not admit myself into a healthcare that is dirty or not cared for. I would also avoid a hospital or facility that is in violations of regulatory requirements, HIPAA violations or has great employee turnovers.
