The IDT then coordinates services through regular face-to-face interaction/evaluation with the patient and their family no less than every 15 days. At present HRRV provides care to patients in a 32,000 square mile area in the eastern half of North Dakota and the north western area of Minnesota. In 2015 HRRV identified a inconsistencies in length of IDT meetings. Currently HRRV does not utilized a standardized report format for information shared nor documented. Through the development of an IDT report template, IDT meetings can run more efficiently, provide improved documentation and ease recertification/appropriateness for care determination.
