Differential diagnosis:
Two possible diagnosis:
1. Stable angina: Occurs with trigger from
a. Smoking: Bob smokes 30/day
b. Stress
c. Heavy meal
d. Cold/hot temperature
2. Unstable angina: occurs mainly
a. While resting, sleeping or little exercise: Bob was in work
b. Comes as a surprise
Between these stable anginas is induced by exercise when the demand for oxygen is increased to the heart but narrowing of coronary artery fails to supply sufficient blood to the heart while unstable angina is caused by partial or complete blockade of coronary artery through blood clot.
Here in this case the patient is a suspect of stable angina because work in the factory may have increased oxygen demand for the heart which may have not been fulfilled due to narrowing of coronary arteries. In addition smoking could be another trigger for stable angina for this patient. Also the main symptom of stable angina is the feeling of chest pain at the center with discomfort in jaw which were manifested by the patient on arrival. Also, unstable angina is eliminated because of the occurrence of chest pain was not during resting or sleeping.
So the possible diagnosis of this patient is stable angina.

You have been called to a factory where you find Bob, a 35 year old man, is experiencing central chest pain radiating into his jaw. He is pale cool clammy and obviously distressed.

Background:
The patient has no previous complaints of chest pain.

PMHx:
HT, Asthma, GORD, Type 2 diabetes Mellitis- diet controlled.

Med’n:
Atenolol, salbutamol inhaler, Breo inhaler, omeprazole, paracetamol PRN.

Allergies: Penicillin Comm.

Diseases – Nil Known, up to date with vaccinations.

Social Hx:
Lives with partner Family.
Hx:
Father died of heart disease aged 41, mother died of breast cancer.
