Critics argue that the reason why some terminally ill patients wish to commit suicide is nothing more than melancholia. Patients suffering terminal illness might tend to be negative, hopeless, and depressed. In “When Patients Request Assistance with Suicide,” Dr. Michael Maskin, an associate professor of clinical psychiatry at Columbia-Presbyterian Medical Center in New York, argues that in many cases, dying patients’ thinking is simply occupied by negative reactions to their critical condition (1999). In other words, most of the reasons why terminally ill patients request doctors to assist them in committing suicide might be caused by certain problems related to their illness. For example, they may be experiencing problems such as hopelessness because there is no effective treatment, anxiety over doctors’ very expensive fees, and regret for their family’s burden of taking care of them (Maskin, 1999). For this reason, opponents argue that the terminally ill patient needs psychotherapy. Therefore, people who oppose doctor-assisted suicide believe that these patients are much too depressed to make a logical decision.

Incident command system

The scenario provided is quite complex. The first arriving law-enforcement unit established command down the road from the scene. As the senior fire officer arriving on scene I would take command and begin developing an incident action plan. I would have three primary concerns including life safety, incident stabilization, and property preservation. Most importantly I would like to focus on life safety. According to the scenario we have several passengers on the aircraft who will need immediate medical attention. We would also need the local fire department to begin rescue operations for the motorists trapped in their vehicles on the highway.

There would be several responding agencies including my fire department, the local police, and local EMS agencies. My first task as incident command officer would be to activate mutual aid contracts and obtain as many resources as possible in a short period of time. My fire department as well as local fire department who respond would be charged with rescue operations and fire suppression. Local EMS agencies and other responding agencies would be responsible for triage and treating patients as well as transporting patients to the local hospital. The local police department would begin evacuations of affected areas and traffic/ crowd control. Traffic would be diverted off the major highway and roadways effect iced. I would want local hazmat or regional hazmat to be activated as well considering there is a significant fuel spill. The state police would be activated in order to provide additional resources for local police officers regarding the direction of traffic which would be diverted off the highway and they would be useful in securing the scene. The federal Aviation agency would arrive on scene for investigative purposes after fires have been extinguished, lives have been saved, and property has been salvaged. Regional hazmat teams would control fuel spills and evaluate other chemical hazards based on wind and weather conditions. They may recommend a change in the command post. The DOT would also be dispatched to the incident in order to evaluate damage to the highways and roadways affected. All responding agencies with investigative purposes would be required to wait until lives have been saved and the incident has been stabilized.

All of these tasks would be happening at the same time including the delegation of the positions of safety office and the public information officer. These two individuals would work closely with me because there would need to be constant communication. I would need to make sure that all of my responders are safe at all times and I would need to ensure that the public is informed with the most accurate and up-to-date information. As soon as possible I would begin to develop my incident action plan, most of this plan would be mental, however I would need to make notes to distribute for other responding agencies to understand the plan and their responsibilities. I would use the incident command system structure for my response. Under unified command all responding agencies would be given operational tasks based on the needs of the situation as it develops. The planning division would be responsible for writing down the incident action plan as it develops and bringing new developments to my attention regarding the incident and plans for response and recovery.
